Required Forms
For new patients there are New Patient Forms that must be filled out and submitted. Please visit our Forms page at the link below to print out the forms needed for your visit. You will need to call us for the password. You may return them to us via fax (484-930-1314), mail to 6034 Hamilton Blvd., Suite #327, Allentown, PA 18106 or attach them to an email to us at aimmed420@gmail.com.

Step 1: schedule an Appointment (All appointments are via phone call)
Call us at 484-602-4438 or click on the “BOOK NOW” button to schedule your appointment. We always return phone calls within 1 business day. We encourage new patients to call in order to schedule as there is additional paperwork needed and our new patients often have additional questions.
Step 2: Obtain Your Current Medical Records
Contact your physician and obtain a copy of your current medical records. These documents must include, at a minimum, your diagnosis (that qualifies you for receiving medical marijuana) along with records of your current treatments and medications.
These documents may come from your primary care physician or specialist. An example would be a copy of your office visit with your Neurologist that documents your diagnosis of Multiple Sclerosis or from your Gastroenterologist documenting your diagnosis of Crohn’s disease and your current treatment. A discharge summary from your physician may suffice if it includes your diagnosis and medications.
For patients with PTSD or Anxiety Disorder, an office note or letter from your Psychiatrist, Psychologist or Licensed Therapist would serve.
If any of your doctors are associated with any of the major health networks you can access your records through their patient portals. Print a copy of your “Health Summary” - usually found under the “Menu” button - and send us that.
These records must be received by us before you can be certified for the State’s MMJ program.
IMPORTANT NOTE: you can have your appointment without your required medical records but you cannot be certified until we receive your records.
Step 3: we will call you at the time of Your Appointment
Have your credit or debit card available at the time of your appointment.
Allow adequate time for your visit.
Patients under the age of 18 must be accompanied by their parent or legal guardian.
Step 4: register with the pa medical marijuana program
Visit the Commonwealth Of Pennsylvania website for medical marijuana patients and follow the instructions to create a patient profile in the patient and caregiver registry. If you are registered before the time of your appointment your certification will go much faster.
NOTE: We have a 48 hour cancellation policy. If you cancel after that or are a “no show” for your appointment then you will be charged a 50% non-refundable fee.
